In our office, we offer high quality care and payment options that work for all of our patients. Below you will find a list of some insurances and payment options that we accept. If you don’t see your insurance or preferred method of payment on our list, please call our office at Cosmetic & Natural Dentistry Phone Number (610) 252-1454 to find out if your plan is a part of a bigger network we participate in or what other options may be available to you.

Dental Insurance PPO Plans We Work With:

In office Membership Plan

               ANNUAL MEMBERSHIP FEE IS $150.00

        ADDITIONAL FAMILY MEMBERS JOIN AT $75.00 EACH.

       YOUR MEMBERSHIP WILL INCLUDE:

       2 DENTAL CLEANINGS, CHECK-UP FILMS AND EXAMS FOR: ADULTS  –  $65.00 (REG. $362), CHILDREN – $60.00 (REG. $327).

        ANY ADDITIONAL CLEANINGS OR PERIODONTAL MAINTENANCE (WITHOUT EXAM) WILL BE $75.00.

       PROBLEM FOCUSED/EMERGENCY EXAM INCLUDING A PERIAPICAL X-RAY – $65 PER VISIT (REG.$141).

                        15% OFF ALL DENTAL TREATMENT

       MEMBERSHIP DISCOUNT CAN NOT BE COMBINED WITH ANY OTHER DISCOUNTS.

Payment options:

• Cash: Always accepted.
• Credit Card: Visa, MasterCard, Discover, or American Express.
• CareCredit: Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for any co pays and the fees not covered by your insurance. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is also your responsibility.
